fix: rename Macaly Code to Macaly Cloud - #6

The product is now Macaly Cloud. The ChatGPT and Claude MCP configs used the deprecated /api/code-mcp endpoints, which Macaly will remove. They now use /api/cloud. The MCP server key is now macaly-cloud, the same as the universal config. The plugin ID macaly-code stays, so current installs and install links continue to work.
The Claude endpoint now has 20 tools. The skill and the submission doc described the older set of 14. - The skill covers edit_file, read_file line ranges, list_projects with get_project for existing apps, media through upload_file and list_media, preview_app at the end of every turn that changed the app, liveUrl and connect_domain. - The submission doc lists all 20 tools with their current annotations. preview_app is not read-only, and upload_file and connect_domain were missing.
